Community Mental Health (CMH)
CMH specialized program, JPORT created in 2007 by the County, is contracted to provide mental health and psychiatric services and continued care into the community to the inmate population within the jail. JPORT is an integrated mental health/substance abuse treatment team that is based on Minkoff's best practices co-occurring model and the Sequential Intercepts for Change Model (SIM). Together the Sheriff's Office and JPORT identify inmates with mental health issues and provide assessments, treatment, groups and discharge planning to support successful community re-entry. JPORT efforts have shown significant decreased jail days of this population and an increase in quality of life.
