Disease Reporting Forms
- All diseases (except HIV/AIDS): To report a communicable disease, please complete this secure online disease reporting form. As an alternative, a Disease Reporting Form (PDF) can be printed and faxed to 734-544-6706.
- HIV/AIDS: Print the reporting form (PDF) and, if needed, form instructions (PDF). Mail to Washtenaw County Health Department ATTN: Christina Zilke 555 Towner Ypsilanti, MI 48198.

- Animal Bites or Bat Exposures: To report an animal bite or bat exposure, please complete this secure online animal bite reporting form.
School Report Forms
Schools and childcare settings are required to report cases and outbreaks of communicable diseases to the Health Department. Please use these forms for reporting:
- Use this form for outbreak reporting: bit.ly/outbreakreport. This form should be used anytime camps/schools are aware of an outbreak that exceeds 20% of a particular group (e.g., a classroom).
- Use this form to report communicable disease cases. This form should be used anytime you have a case of any communicable disease where reporting is required with even a single case (e.g., measles, pertussis). These require immediate reporting. Foodborne Illness Reporting
If you believe you have gotten sick from food you ate at a grocery store or restaurant in Washtenaw County, please go to the Environmental Health site for information on how to report.
HIPAA Guidance Regarding Disease Reporting
