Post-Adoption Services: Seeking Identifying or Non-Identifying Information
Adoption agencies, the Michigan Department of Health and Human Services (MDHHS) and Courts may release certain information from adoption records to adult adoptees, former family members and adoption parents of a minor child. The law divides the information in adoption records into two categories: Non-identifying and identifying information.
If you are seeking identifying or non-identifying information from a closed adoption file, please file the notarized Petition for Adoption Information and Order (PCA 327) with the Juvenile Court, along with a copy of your photo identification and the $20 filing fee made payable to Washtenaw County Trial Court. If you are seeing a certified copy of the Order of Adoption, please include an additional $11.
If you are requesting a Confidential Intermediary be appointed to search for, and potentially make contact with, former family members, please file the Petition and Order to Appoint Confidential Intermediary (PCA 342) with the Juvenile Court, along with a copy of your photo identification and the $30 filing fee made payable to the Washtenaw County Trial Court. The Confidential Intermediary charges $300 for this service and will communicate with you directly regarding the transfer of these funds after he/she is appointed. A Statement of Understanding and Fee Agreement will be sent to you upon his/her appointment.
