Reporting COVID-19 Cases

Schools and childcare settings are required to report positive COVID-19 cases (as well as other communicable diseases) to us. 

• Use this form for weekly reporting: bit.ly/schoolweeklyreport. This form should be submitted every Friday by 5pm. 
• Use this form for outbreak reporting: bit.ly/outbreakreport. This form should be used anytime camps/schools are aware of an outbreak that exceeds 20% of a particular group (e.g., a classroom).
